How the program works:

Hearing Care Services

How the program works:

Laser Vision Correction and Contact Lenses

$895 per eye, including pre-operative exam, LASIK or PRK procedure, postoperative care and retreatment warranty

100% patient financing is available upon approved credit through TruVision.

How the program works:

Call TruVision toll-free at 866.339.2020 to be screened for the procedure and get answers to your questions.

If you pass the screening, TruVision will:

• gather your information

• provide you with a doctor based on your location

• collect a deposit for your exam (generally between $100-$150 per eye) OR approve financing

• schedule pre-operative exam

At your pre-operative exam, you will need to present your ID card. The selected doctor performs a comprehensive exam to determine if you are a candidate for laser vision correction surgery.

• If you are a candidate, you will be scheduled for surgery.

• If you are not a candidate, your deposit will be refunded.
   

If you are paying in cash for the laser correction procedure, the clinic will collect the balance due prior to or on the day of surgery. If you have financed the procedure through TruVision, the payment will be paid to the laser center automatically after the procedure has been performed.

The doctor performs the procedure and honors the following Re-treatment Warranty:

• TruVision has contracted with surgeons and their facilities to waive all professional and facility fees in the event a re-treatment is necessary.

• The only cost that is not covered is the equipment manufacturer-licensing fee. Depending on laser used, the cost for this could be $110 - $150 per eye.

• Re-treatment is offered for life provided that you schedule an annual re-treatment exam through TruVision with a participating provider. The cost for this exam is currently $60.

• You will be reminded of the annual re-treatment exam via mail and email.

Vision Discount Program

We offer a Vision Care Discount Program to all of our members through Davis Vision, Inc., an independent company and national administrator of vision care programs. You may choose from Davis Vision’s contracted private practice providers or contracted retail locations for discounts on eye examinations, frames, lenses and contact lenses.

We also offer a variety of BlueVision™ vision care plans with more extensive coverage.

How the program works:

To receive eye care services through the Discount Program or a BlueVision plan, call the Davis Vision network provider of your choice and schedule an appointment. Be sure to identify yourself as a Davis Vision plan participant and a BCBSD member.
Please note that all existing vision benefits, if any, associated with your health benefits plan still apply.
