BLUEPRINTS FOR HEALTH - Care Management

 

 

 

Care management programs supply you with valuable information and services, which, along with a doctor's care, can help minimize or prevent complications related to chronic conditions and enjoy a better quality of life. These early detection and lifestyle improvement efforts are also designed to reduce inpatient admissions, hospital days, emergency room visits and outpatient facility visits.

Our care management programs include:

Basic Chronic Conditions

  • Asthma

  • Chronic obstructive pulmonary disease (COPD)

  • Congestive heart failure (CHF)

  • Coronary artery disease (CAD)

  • Diabetes

Rare and Complex Chronic Conditions

  • Amyotrophic lateral sclerosis (ALS)

  • Chronic inflammatory demyelinating polyneuropathy (CIDP)

  • Cystic fibrosis

  • Dermatomyositis

  • Gaucher’s disease

  • Hemophilia

  • Multiple sclerosis

  • Myasthenia gravis

  • Parkinson’s disease

  • Polymyositis

  • Rheumatoid arthritis

  • Scleroderma

  • Seizure disorders

  • Sickle cell anemia

  • Systemic lupus erythematosus

How Care Management Works
Eligible participants are identified through claims and pharmacy data, physician referrals, case management nurse referrals, referrals from BCBSD’s utilization management staff, and self-referrals. If you are identified as a having a specific health condition, you will be automatically enrolled in the appropriate program and will then have access to:

  • A 24-hour information line staffed by registered nurses who specialize in chronic conditions

  • Educational resources, including monthly newsletters, online articles and health materials

  • Outreach calls by well-trained, experienced health care professionals

  • Support services and information about local and national resources

  • Information about their conditions, course of illness, complications and therapy

  • Assistance with behavioral changes to foster self management skills

  • Coordination of health care services such as durable medical equipment

Participation in one or more care management programs is optional and you may choose to opt out of the program(s) at any time.
